What to expect before your first visit
Your New Patient Form
You can print the form and fill it in at home, or arrive 10 to 15 minutes before your first appointment and complete it at the office. It covers your contact details and your health history.

Bring your insurance card if you would like us to submit the claim on your behalf.
New Patients
Many of our patients are referred by their dentists. New patients are welcome by appointment. Arrive 10 to 15 minutes early to complete your new patient form, and bring your insurance card if you would like us to submit claims for you.
Returning Patients
Let the front desk know if anything has changed since your last visit, including insurance details, medications or health conditions.
